Injury Reporting - Serious Injuries: Call 911 immediately

Call EH&S Serious Injury Hotline at x59797 as soon as possible.

General workplace injuries (not serious):
Employees shall inform their supervisor of the injury, and then fill out the Incident Report and Referral for Medical Treatment Form and the DWC-1 Claim Form. Employees shall report to the Occupational Health Facility for treatment (CHS 67-120). EH&S Facts - Reporting Workplace Injuries
